How can this be done?

A faceless person, you can just barely see the place where eyes/nose/mouth should be.

It's like he has skin over them and a flat nose.

Can this be done in After Effects?

What would be the steps?

If it can't be done in AE, what can it be done in, and what would be the steps.

I think you would be better off coming up with a practical solution, like a latex mask. The alternative is motion tracking the face, creating the faceless features, and moving them as the head moves on the XYZ axis.

Yeah, I saw the BBC do just that on an a David Tenant era Dr. Who episode just last night. It was makeup, latex appliances and whatnot. In other words, practical effects and not After Effects.

I love practical effects and given the change I'd always go for that, but cost is a factor that might have you go the AE way.

That being said, I might do it practical afterall...

You can use FreeForm Pro or Free Form V2 to create a three dimensional face. Tracking and replacing the face on the actual person may be a challenge, but it can be done. You can isolate the face in a comp and do a 3d camera track like in this tutorial:
