Masking on multiple layers

I have seen this covered before on CC, however, not with the issue I am having.

I need to create a mask that covers all layers under the top layer.
I have tried to use an adjustment layer, but it just isnt working for me.

Basically, whenever I create a mask, it follows the items around, so its not a true mask. See the attached image for what I am trying to mask.

Hope someone can help. Its probably the easiest thing, but I just can't work it out.

The stencil and silhouette alpha blend modes are also good options. These would be applied to a solid layer above the layers you want to mask.
