I'm trying to figure out the best way to set up an AE comp for a portrait monitor. Can someone direct me to either a site that explains the correct settings or tell me what they used.
You would think it would be set it up 72x1280 vertical then use a 1280x720 comp to rotate it. But when I do that it doesn't fit correctly on the monitor. Is it something to do with square pixels vs D1/DV wide screen pixels?

The video signal going to the portrait monitor is still 1280x720.

You can work in a 720x1280 comp, but for output, nest your portrait comp in a regular landscape comp and rotate it. You'll need to know which way to rotate (clockwise or counterclockwise) to match the display, or just render it both ways to be safe.

But does the first comp need to be square or D1/DV NTSC widescreen (1.21)?

D1/DV NTSC Widescreen (1.21) is only for 16:9 SD.

HD should generally be delivered square-pixel, but you can check with the folks handling the display to be sure.
