Projecting Flames onto Live Action

Hello all, just a question in regards to 3DS and FumeFX. Firstly I should point out that I have not used 3DS Max, but am ready and willing to learn. I caught onto After Effects fairly quickly, so I feel confident that I can take on the big 3D.
Anyway, I am wondering how to create a 3D rendering of fire (or related fire FX) to add into my footage. I imagine some motion tracking and 3D modeling is involved, but specifically what steps?
For instance, if I want to catch a hand on fire, do I need to just motion track the hand/footage, or do I have to create a hand model in 3DS to attach the fire to and then key out later? Or both?
