Random mismatched clips in Resolve timeline

We are having an issue where we send an Avid timeline to Resolve via AAF, and when we import the AAF in Resolve, about a third of the clips are mismatched to the wrong footage and have the caution symbol below them. When we bring up conflict resolution, the correct clip is there and after selecting it all is good (though we have to do this manually for every mismatched clip). The mismatched clips are media from the same project, but in no other way share similarities with the clip they're overriding. Different clip name, file name, and length. It seems completely random. Any idea what is going on? This has only happened once, just today on a short promo we are going to grade.

Here's our workflow:

- Import footage into Avid as DNxHD .mxf
- Edit
- Consolidate to a finishing drive that is empty and has no other .mxf media on it, only the consolidated media from the edit that will be graded
- Export AAF file for round-trip to Resolve using the "link to footage" setting
- Import AAF into Resolve, directing towards the finishing workspace on ISIS to find footage
- Grade
- Deliver via Avid Round-Trip preset, exporting graded .mxf files to ISIS finishing drive
- Import new AAF that Resolve generated into Avid project
- Consolidate graded footage so it belongs to the avid project
- Final output

So far this workflow has been working with very few issues. This is the first time we've run into randomly mismatched clips. Any idea why this might be happening?
