Shot log?

Is there a standard method of writing down camera shots (and other info needed on location) before the shoot? I have a a complex 5 min. video that involves a lot of compositing coming up on the other side of the country. I have to make sure I get everything, no re-takes possible. I have always used my own method that seems to work OK. (Excel spreadsheet with the type of shot, camera angle, type of lighting etc.) I am sure there is a better method than what I have been using. Is there a standard format used for this type of thing?

you can print a boat load of blank ones. I'd prolly not fill it in till I start shootin, though.

I am looking more for something to keep me organized for the shot. Something like a detailed storyboard.

What are the blank sheets you are referring to?
