Lighting a medical movie

I am shooting a scene in a patient exam room. The usual place with no windows and overhead fluorescent lights. I plan to turn the fluorescent lights off and light to room with tungsten lights to simulate the basic overhead exam room effect but to add shadows and highlights to make the effect a bit more defined and dramatic but without out losing the basic feel of the examination room. My cast is all African American so I need to be able to create reflections when illuminating faces and not crush everything to black. It is a family meeting with eight people in a small room. I am recording to DV and editing with Edius Pro 3 so I have both a limited pallet with DV but good color adjusting tools with Edius.

Without further guidance I plan to use a lantern to provide overall room fill light and to light individuals with an Arri Fresnel as the key and a white bounce card as the fill. To light the whole room, I guess I'll use the lantern as fill and provide another cross fill with a big soft box.

One guy is 6'6" in an 8' room and I can't figure out a hair light or what to do with the boom microphone.
