Kona 2 Installed - Test thinks is missing hardware - ???

We still have one or two systems running with Kona 2s. These are almost 5 y/o G5 edit suites that have ran every day without any problems. Yesterday, we had to capture some tapes and for reason the Kona 2 had major problems. No HD-SDI was detected. DVCPro HD decks were not detected but HDCam decks were.

The weird thing is that when running the self tests the system did not recognized the card. It prompted a message of "missing hardware".

I install the new Kona release but that didn't help. It wouldn't run thinking the hardware was not even installed. We completely disassemble the G5, change PCI slots, change cables and nothing." The card is just busted, I thought" And the idea of buying a complete new system didn't let me sleep last night.

Today, I did a complete uninstall of all Kona drivers, downloaded the previous Kona software from 2008, ran Drive Genius, defrag the entire system, change the card back to the original PCI slot, install the 2008 release, connected the break out box and everything works!

So far it is working but I am keeping an eye on it in case everything screws up again.
Has anybody encountered this type of random issue?

Which slot was it in?

And did anyone bump the back of the machine, move any cables, reconnect anything, etc..? I've seen multiple cases of the cards just coming a little loose inside so all the connections are not there.

There is no access to the machine from the back and it's pretty much impossible anyone can get behind it . All cables and card were very secured, nothing was loose (that was the first things I checked).

It is working as it suppose to be right now.
I guess it was just one of those days....

Similar thing happened to me today. I have an old G5 running a Kona 2
on 10.4.11. It's still running fine on 6.0.2, but things got messy when I tried
installing 6.0.3. The card wasn't detected etc... I uninstalled 6.0.3, re-installed
6.0.2, restarted the computer each times and repaired permissions. It's now
running fine again... on 6.0.2.
I guess the forgot to run tests on the K2 B4 releasing the new driver...
