How do I figure out ARRI LUT generator to dl Alexa classic 709

My computer has been stripping my dp's camera LUT settings so I have to go back in and manually reattatch the LUT to my footage. The problem is my DP doesn't know how to send me his LUT and wants me to download it from the ARRI website. I'm not sure what inputs I should put into the generator in order to get an Alexa classic 709 thats appropriate for my footage and Premiere project. Does anyone know how to do this?
My footage is Pro-res 422. I know the source format is Log C but otherwise I don't have a clue.
