Stereo and 5.1 mix sounds fine in timeline but not in quicktime file

This quite possibly might be a dumb question but I really need some insight.

I am exporting a file for delivery and the specifications are below. They need the following:

Track 1: Stereo Left
Track 2: Stereo Right
Track 3: M&E Left
Track 4: M&E Right
Track 5: Left (5.1)
Track 6: Right (5.1)
Track 7: Center (5.1)
Track 8: LFE (5.1)
Track 9: Left Surround (5.1)
Track 10: Right Surround (5.1)

I set up my sequence accordingly utilizing a multichannel sequence( mapping each channel and panning left/right). Everything plays just fine in the timeline in premiere pro.

Upon export I rendered the file out as an apple ProRes 422 (HQ) file with uncompressed audio. I selected 1-4 as my first stream and my 2nd as the 5.1.

In premiere pro everything sounds great but the quicktime sounds like it is phasing...?

My other deliverable set up is as follows:

Track 1:Left (5.1)
Track 2: Right (5.1)
Track 3: Center (5.1)
Track 4: LFE (5.1)
Track 5: Left Surround (5.1)
Track 6: Right Surround (5.1)
Track 7: LT
Track 8: RT

I handled the export the same way except for this one I choose SMPTE DTV (L R C LFE LS RS LT RT) based on the set up they requested and upon export the quicktime plays fine with no sounds of weird phasing....

Any help or information on this would be greatly appreciated.

I have attached some screens for reference.
