Audio lags behind video by two frames.

I recently discovered that any video I bring into premiere is two frames out of sync with the video leading the audio. I have tested and confirmed this using Catchin' Sync: http://quietart.co.nz/catchinsync/

VLC is in sync. Quicktime is in sync. It's just Premiere.

All the audio waveforms line up as they should. When I scrub through the timeline, everything seems fine and in sync. But when I hit play, the audio lags ever so slightly.

Anyone have any thoughts on what could be causing this or how I can fix this?

Did you create your sequence using the settings from one of the clips?

Yes, Dave.

Whether a clip is in the timeline or source monitor, I have this problem. It doesn't matter if the clip originated in Premiere or outside of Premiere.

What happens if you export? Is the audio still behind the video?

If I export a clip and play it in VLC or Quicktime, it plays back correctly. If I bring that exported clip back into Premiere, it's two frames out of sync.

if you transcode the variable framerate video with handbrake, the problem should go away.
mediainfo should tell you if its variable framerate or not.

It's not a variable frame rate video. I've already checked.

if you transcode to prores, cineform, or dnxhd, does the problem go away?

Unfortunately not. I've tried with H264, ProRes and DNxHD.
