Questions about converting html forms into a flash version

So I'm trying to create a form on my flash page which sends some information using the post method, but am unsure how I can recreate this in flash. If anyone can give me a quick lesson in creating / coding forms in flash or point me to a website with some information I would really appreciate it. Below is the raw information I would use if I created it in html. There is a space after every < to allow the code to be visible on the post rather than showing the rendered out html.
