Not able to insert edit on D5 - off and on problem

I've had this problem off and on but looking for a solution. I can assemble edit fine, but not insert. The button is greyed out. Switching genlock from freerun to ref in to video in makes no difference. Switching to Panasonic 422 protocol from Sony 422 made no difference either.
Any ideas?
