Sony JH3 rejecting an HDCAM tape

Any ideas why our jh3 deck is ejecting/rejecting a 1080PsF 23.98 HDCAM tape?
The same tape works fine in our SRW5500 deck.
The tape is definitely an HDCAM not an HDCAM SR.
Other 1080PsF 23.98 HDCAMs are playing fine in the JH3. its just this one tape.
I've examined the tape shell for any unusual obstructions or punch outs that are bad. I see nothing out of the ordinary and it looks identical to other hdcams we have on hand.

The grouping of holes is what I was referring to as "punch outs." The ones on this tape are indeed identical to other HDCAM tapes we have. Before posting I even tried taping over the ones that are not punched out in case the deck was reading them as punched out for some reason and that didn't make a difference.
Flipping to view from the underside, and with the tape flap facing away from me, the holes/punch are between the flap and the reels, dead center. The only one punched is the one on the south east quadrant. This is identical to several other HDCAMS that play fine. I also compared it to an HDCAM SR tape and Digibeta which have clearly different punch out configurations.
