Steve,
In MOTU Video Setup, choose the SD or HD SDI input as the source.
In MOTU Audio Setup, make sure SDI input is enabled. Make sure whichever outputs you are using are enabled.
In CueMix, set up the input to output mix.

Yes, the V4HD can embed or de-embed audio. The V4HD has 32 channels of audio I/O. This consists of 8 channels of analog, 8 channels of optical, 8 channels of AES/EBU, and 8 channels of HDMI/SDI embedded.
