There's a subtle, yet important aspect of this effect that I don't see listed here, it's the way the text blurs and then fades at the top, pretty easy to miss or NOT do correctly.

That aside, there is a little free app called SWTSG which is absolutely amazing, you simply enter your text and it's done. Here's the link:
