Check your item properties for the clips, the audio and the sequences. I think you have a time base problem.

I've seen this with students that use advance pulldown removal. Sometime they create a clip where Quicktime sees it as 23.98 and Final Cut sees it as 29.97, and it causes all kinds of sync issues.

You may have an audio sample rate mismatch, bit rate mismatch, or sequence timebase mismatch of some kind.
