Absolutely doesn't.

1. Shoot the subject ensuring that he is as still as possible.

2. Composite the eyes, mouth, and a still of the thumb, as above. Four layers.

3. Nest those four layers, then animate the whole body as needed.

It's not elegant, but neither is the effect.
