Simple Grid in Trapcode Form?

Hi, I'm wondering if there is a way to create a very basic, simple line grid in Form. something like this http://grail.cs.washington.edu/projects/digital-matting/shadow-matting/imag...

my goal is to have this plane constantly receding into the background as if a camera were being pulled backwards, and synch some audio to the grid to see some vibrations/distortion of the lines.

Please if anyone can help out w/ suggestions of any kind it'd be much appreciated.
