It does. AJA Io Express (I/O) and Matrox MXO2 Mini (Output only).

Yes. But you get one HBA with the Io Express, so you'd need to purchase the other one separately. There's a PCI HBA for your workstation and there's an ExpressCard HBA for laptops. Make sure your MBP is one that has an ExpressCard slot.
