I haven't had my hands inside an Avid since 2003 or so, but here's some rusty free info:
If you have the PCI extender, chances are you already have the Avid cards in the extender. IIRC, the cards are the Avid DMB (digital media board) which has two connectors that look like the SCSI ones parallel to each other one marked M the other marked C (connect the BOB to the M); and the EDC card which has two db9 or vga connectors on it, covered with black plugs on most systems I've worked with. You'll also have your Avid monitor card, for your edit side monitor, which connects to the DMB internally with a ribbon cable.
The card and cable you REALLY need to connect to the G4 is the PCI Extender card & cable. The plug on the card is pretty wide as is the thick ribbon cable that goes to the PCI Externder box.
Also, IIRC the ATTO Ultra SCSI card should go in the G4 directly - look on Avid's site or inside the G4 for which PCI slot to put which card in.
