Neil,
Things to try:

Factory reset. This is done from the front panel of the V4HD.

Disconnect all other Firewire devices from the computer and connect the V4HD directly.

If possible, try connection to a different computer.

If none of this helps, you can call MOTU at 617 576 2760 and ask for a V4HD technician. The tech can double check and if needed, arrange a repair/replacement.
