I'd heard one reason for this happening in the UK (BBC now don't take S16 originated material) was due to grain - the compression that they apply for our digital terrestrial service (Freeview) freaks out over the grain, so they can't get high enough compression to deliver an acceptable picture whilst still offering all those poxy interactive 'red button' features that use up all the bandwith. Dunno how accurate this is but it came from a good source.
