From what I understand, iTunes approves pretty much anything that is in the right format. They do not make artistic evaluations and leave that up to the market to decide what is good and what is bad. All they care about is that the RSS is done correctly and that the file plays when called.

You can submit with the first episode, you do not need a pile to begin with.

Approvals used to be a day or two long ago but today it seems to take a week or so. There are a lot of people getting in line and the iTunes approval staff is not huge.
