the field order is dependent on what you will be working in. DV is usually lower field first. HD is always upper field first.

Lossless CODECs like Quicktime Animation are usually the best bet to get work from one application to another. Also image sequences, where each frame of video is output as a separate numbered image that can be re-compiled into video when imported.
Mostly it will depended on what Premiere is capable of importing.
