how to create an order form with dreamweaver

i want to create a simple order form using php and mysql with dreamweaver, on the form i want to display the products by a dropdown then when you choose the product, show appear the information such as preice opcion to insert the quantity and a total as a result from price x quantity, in the buttom a case showing the total.... i am new in this, i have search in the internet, but they hace a all complicated procedures like login, autentizication, and so on, i only want that form...... ahh i know how to make a database connection an so on, in order to save that part
