My personal work flow is this:

First comes the site structure (menus, site map)

Then it comes the content (HTML)

Then it comes the layout (CSS)

Then it comes the aesthetic design (CSS)

Then it comes the templates

Most of this first happens on paper, then Photoshop, then Dreamweaver.

I hope this helps
