If you are using Dreamweaver 8 follow this:
1) place the cursor where you want the flv player to be embedded.
2) click on Insert menu and choose Media and then Flash Video option from the pop up menu.
3) from the resulting dialog box choose the type of video either progressive or streaming. In the url field give the url of the flv video file or use the browse button. Similarly all the other options are self explanatory and choose according to your needs.
4) at the end click OK, and the source code will be generated along with some other .swf files
5) test your file.
Note: the source can be from database, but requires that how much control you have on the Dreamweaver
