HDV TC Breaks, not digitizing all of tape

we get an HDV project from time to time, 90 percent of our footage is 24p DV footage so we like to do a down convert with the HDV footage to DV, since it might end up in a DV project down the road. This time around every tape has been giving us 20-30 TC breaks. and Sometimes just sitting at one point in the tape and digitizing the same 5 second clip over and over again.
The tapes have very few TC breaks in them to start with, maybe 5-6. sometimes less
my troubleshoot list includes
Digitize to Our San
Digitize to External G-Raid
digitize from deck and from Camera
no Down-convert-just straight-HDV ingest
FCP 5.1.4 & FCP 6
Clean the heads
have tried multiply tapes
Dumped Prefs
Tried multiple edit stations
We digitized one of this producers HDV project a few weeks back with down-convert and had no problems what so ever, same deck, might have been different camera though.
Well if you have any insight on why some many TC breaks and why it keeps getting stuck at one point that would be great. thanks in advanced, also posted in the HDV forum
