John,

For the VGA issue, you must check that the separate VGA cable is connected from the back of the Altitude card to the VGA input on the BOB. Or you can use that cable straight to your VGA input on your monitor (bypass the BOB). This cable has a 9pin mini-DIN connecter on one end and a typical male HD15 on the other.

As for SDI input on the VHD, you should be using SDI-input 1 on the BOB. Your capture settings seem to be correct.
