There are few common causes to the problem:

1. Not using AC-3 sound
2. Not using the most reliable (and dare I say most expensive) media
3. Not using CBR for the encoding
4. Trying to play a DVD in Windows Media Player on a machine that can't decode MPEG2 (PC/Windows machines _CAN'T_ do it by default) and that doesn't have a dedicated DVD-playing software installed.
