I think you have 2 choices here....

1. you can use an overlay graphic to cut the hole for the button highlights... or

2. you can use a DVDSP "shape" to cut the hole.

Simply drawing the button box will not create a highlight.

As for setting the "Title" menu... you can set this in the disc inspector.
