Actually pretty simple to do...

1) Put a Reorder Node after your depth File-In, then put "RGBAR" in the channels input (this tells Shake to create a Z channel from the Red channel of your grayscale image)
2) Then put a DepthKey Node after that
3) Then put a Blur Node after your main image File-In
4) Then just connect the output noodle of your DepthKey to the (Right) Mask input of the Blur node and adjust the Blur Node and DepthKey parameters to your liking.

(I'm basically still a Shake newbie myself; I can highly recommend working through the included Shake tutorials...Chapter 3 (p.91) has a whole tutorial devoted to Depth Compositing.)
