Does the DVD player offer any other outputs? Composite or Component for example. Have you tried with those at all? Am just wondering whether the issue is HDMI specific or something else. Is your MXO2 LE new or have you used it successfully before? Have you captured via HDMI from other sources without issue? And have you captured via SDI, Component etc? You note that you are capturing from DVD (so we are talking about an SD signal) and that "Scale input for capture" is not enabled. What is the specific capture preset you are using in FCP?

EDIT: Doh! SD capture via HDMI is not supported ... there's your problem I think
