Need help posting a QT video

I'm completely clueless to putting video on the web so I apologize if these are stupid questions. I want to embed video into a separate page on my site and I've been trying to follow the tutorials with no success. These are the steps I am doing. Can someone tell me where I am going wrong?

1. Used QT Pro to 'Export for Web' which produced a folder with a few different versions of my movie and a reference file.
2. I uploaded that folder to my public_html folder for my website

Now this is where I get lost. I've done this once in the past just like this and all I had to do next was type in the browsers address bar like this - http://www.website.com/video.mov - and it took me to the web page and the video played. That is not working this time. There is a readme file in the folder QT Pro created that says I need to add some code to my web page and it gives me the code and code to use.

Now my questions are...

1. Where do I add this code? Since I want this video on its own page with nothing else, do I create a separate html file named exactly as the movie? Or do I add this code to an existing page?

2. And where do I upload this html file to? To my public_html folder or to the folder QT Pro made which is inside my public_html folder?

I've tried this 100 different times with no success. Can someone help me? I'm sorry if this is a newbie question.

The furthest I've gotten so far is when I created a separate html page named exactly as the video. I uploaded that to my public_html folder. When I do that, the page loads with the poster frame but when I click play, I get the QT logo and a question mark. That's in Safari. In any other browser, I get a blank white page.

Any ideas? Thanks!
