This Kona 2 card never ever seems to work

I'm in a panic
I'm hoping someone can help me!!!
I am using an Aja Kona 2 card and K-Box with the following set-up

Apple Dual 2.5ghz G5
Final Cut HD 4.5
1.5 gigs of ram
UL4D Atto SCSI card
Huge media vault 2.5 TB storage array
JVC DT HD/SD monitor
2x 23 apple displays

I am using the correct 1.0.5 release of the Kona software

The kona card is in the correct slot along with the scsi card

I've tried digitising from three machines

HDCAM HDWM-2000
Digital Betacam DVW-500
DSR-2000

The picture on the HD monitor and the computer displays has pink horizontal and vertical lines running through it.
You can just make out the picture. When I switch to SD it has the same lines. When I digitise and playback it has these lines running through the clips.

I've tried digiting in through standard DV with my DVCAM deck and the pictures works perfectly, however when I go to SDI on this deck and go in through the Kona card I get these lines pinks running through it again.

Is it something I'm missing?

Or does this card just not want to work
