I have no idea what the model number is, it's a MultiFormat monitor basically identical to the Sony PVML5 series that a D.P. friend of mine just purchased. They are getting very hard to find and I'm pretty sure you'll have to go directly to a reseller such as WH Platts or ProMax to locate a CRT Panasonic. I would simply call one of them and tell them you are looking for a multiformat SD / HD CRT monitor and get their feedback.
I love the Sony and highly recommend it.
