Make a donut-shaped "heart" object.

Surface it with a Fractal Noise texture (possibly more than one) to simulate smoke.

Give the texture a low velocity.

Expand the scale of the object and increase the transparency (or do an object dissolve) at the same time to simulate the expansion and fading of the smoke ring.
