DVD Case Cover Insert template

Hi, I need to make DVD covers (full sleeve inserts) and am wonderinfg if any of you folks have a link to a good free template. Obviously I'd like one that is easy to use, and will give me fields for the Spine, Front and Back to work with . I'd prefer it in Word, as I'm not very Photoshop- proficient. Does anyone have a good recommendation?

I am using 8 1/2 X 14" paper, with tear-off sides that fit nicely in the sleeve (bought from professional label.com).
